Doane University Student Health Services is committed to providing holistic care to all registered, undergraduate students. The goal of Student Health Services is to keep students well so they are able to achieve their academic and personal goals. In an effort to do just that, newly accepted students at Doane are required to complete a health related history form and an immunization history through the Doane student health portal. All required information can be found under the “my forms” tab of your student health portal at doane.studenthealthportal.com. Students will need to use their Doane email and password to enter the student health portal.
*Consent to treat- The consent to treat form is required to be signed by a parent or guardian AND the student if the student is under age 19. Please make sure both signatures are present.
* Physical Examination- A physical by the student’s physician is highly recommended for all new students and is required if the student will be participating in an intercollegiate athletic program. Sports physicals should be completed after June 1st.
* Immunizations- All students enrolled at Doane University are REQUIRED to be adequately immunized. This requirement is to ensure the safety of our entire campus from potentially dangerous communicable diseases.
Required immunizations:
- MMR (Measles, Mumps and Rubella) 2 vaccinations are required, one after 1st birthday and then between the ages of 4-6 years of age.
- Tdap (Tetanus, Diphtheria, Pertussis) Within the past 10 years.
- HBV (Hepatitis B Vaccine) Series of 3 shots over a 4-6 month period of time.
- Menactra A,C,W,Y (Meningococcal Vaccine) 2 doses are required if the first dose was received before age 16. The second dose MUST be at age 16 or LATER.
- Varicella (chicken pox vaccination) only if a student has not had the chickenpox virus. 2 vaccinations are required for compliance.
* Insurance- Students are requested to have adequate health and accident insurance coverage as a safeguard in the event of a major illness or accident. Athletes are REQUIRED to have coverage. Doane’s accident insurance policy on the general student body provides SECONDARY coverage for accidents only. This will not replace your primary coverage/insurance deductible, and only comes into effect after your own insurance is utilized. Restrictions do apply.
